Is it possible to set a default .mxd to be opened when ArcFM Viewer for ArcGIS Engine is opened.

I'm not sure about getting a default .mxd but you could write a customized extension to start up V4E with a default stored display.

Can you point me to a code sample that demonstrates creating a custom extension for V4E or point me to the correct objects to use to implement the custom extension.

Thank you

From the command line you can type:

<viewer install directory>\ArcFMViewer <name of document to load>

You could create a shortcut for viewer on your desktop to load it as well. Just modify the "Target" in the shortcut properties to have the full path to the .mxd to load. On my machine the "Target" looked like this:

Target: D:\Program Files\Miner and Miner\ArcFMViewer.exe D:\Program Files\Miner and Miner\Test.mxd
